Job Description:

UCHealth, a leading non-profit healthcare system based in Colorado, is seeking a dedicated Transplant Coordinator RN to join our team. This role is critical in providing holistic care to patients before, during, and after transplant procedures. The successful candidate will be an integral part of a dynamic team, working across multiple hospitals and outpatient facilities to ensure the highest quality of patient care in alignment with UCHealth's mission of excellence and compassion in healthcare. Coordinate the entire transplant process from initial referral to post-transplant care. Educate and prepare patients and their families for the transplant process and follow-up care. Monitor patient health status and respond to any complications arising during the transplant process. Collaborate with multidisciplinary teams including doctors, surgeons, and other nurses to ensure comprehensive patient care. Handle logistical aspects such as coordinating surgery schedules and managing organ donor lists. Strong organizational and communication skills Ability to handle high-stress situations Proficient in electronic health record (EHR) systems Knowledge of transplant procedures and regulations Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current Registered Nurse (RN) license in the state of Colorado Certification in Clinical Transplant Coordination (CCTC) preferred
