Job Description Jackson Therapy Partners is seeking a travel Skilled Nursing Facility (SNF) Physical Therapist for a travel job in Longmont, Colorado. Apply now and a recruiter will reach out with more details. /n /n Minimum Requirements /n - Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. - Active Colorado Physical Therapy license required prior to start of assignment. - BLS (Basic Life Support) certification from AHA or ARC may be required; bring current card if requested. - Recent experience working in a skilled nursing facility or long-term care setting preferred. - Experience providing restorative therapy, fall prevention programs, and skilled PT documentation preferred. - Client requires candidates able to work a 32 hour per week schedule and provide services five days per week as described in order notes. /n /n Assignment Details /n - Facility Type: Skilled Nursing Facility. - Shift Name: Days. - Typical Schedule: 4x8 day shifts (32 hours per week) with five days of clinical coverage per week. /n /n Location Highlights /n Longmont offers a balance of outdoor recreation and accessible regional destinations.
